This warranty is not a guarantee of results and does not imply legal contract. It is a customer benefit that we are pleased to offer you, our patient. This warranty is limited to re-treatment with the same type of procedure or appropriate repair or office credit toward further treatment at the discretion of management. This warranty does not cover misuse or intentional damage. It also does not cover treatment that is temporary in nature or is a repair of treatment done elsewhere or that is an alternative to the recommended treatment. Treatment by a dentist outside of our office that is not recommended by us will void the warranty. The management will evaluate any dispute regarding this warranty and the decision of the management will be final. This warranty is not assignable and has no cash value. Close Window
